Non-emergency transportation

WellSense provides rides to Medicaid-covered medical appointments for our New Hampshire Medicaid members through our non-emergency transportation program. The following types of appointments are available for this benefit:

  • Medical appointments
  • Behavioral health services
  • Vision appointments
  • Prescription pick-up following doctor appointment
  • Methadone dosing or administration of other medications at a methadone clinic
  • Medicaid-covered dental appointments for members under 21 years of age
    • Members 21 years of age and older should review the DentaQuest Member Handbook to learn more about their transportation options for Medicaid-covered dental appointments.

To request transportation for a member’s eligible medical visit:

  • Call 844-909-7433 at least two business days before the appointment.

  • Have the member’s WellSense Member ID number, phone number, address and appointment information available when you call. A Customer Service Representative from our transportation partner, MTM, will help you arrange a ride.
  • If the member is mobility impaired and requires special accommodations, please complete and submit this mobility form.
    • Any healthcare professional working with the member can complete the form, including a health plan care manager or nursing facility staff.
    • The form is valid indefinitely and can be modified at any time by submitting a new form.
