Congestive Heart Failure Care Management

Congestive Heart Failure (CHF) is a serious condition where the heart is weak and can’t pump blood the way that it should. Heart failure does not mean that the heart has stopped, but that it’s working slower and harder to deliver enough blood and oxygen to the body.
