Congestive Heart Failure Care Management
Congestive Heart Failure (CHF) is a serious condition where the heart is weak and can’t pump blood the way that it should. Heart failure does not mean that the heart has stopped, but that it’s working slower and harder to deliver enough blood and oxygen to the body. There are a variety of treatments available and our care management program will help members learn more about CHF and how to live with it. Here are some sources of information you may find helpful:
