After you complete a healthy activity, you'll receive your over-the-counter (OTC) Rewards Card in the mail; please allow up to 30 days for processing. This card is for all members of your household and will be addressed to the member listed as the head of household or your parent/guardian. Keep your card – funds will be added for each healthy action you complete.

Members can earn a maximum of $250 in cash and non-cash goods and services each State financial year which runs from July 1 to June 30. Incentives that apply to this limit will be marked with "*".

You can use your rewards dollars in a couple of different ways, including paying for utility bills such as internet or electricity, and buying over-the-counter items. Your card can be used to pay for items that qualify at these stores:

$25 reward for receiving a cervical cancer screening*

Members can earn this reward once every 12 months. Schedule and complete a Pap smear with your PCP or OBGYN.

YMCA Tai Ji Quan™: Moving for Better Balance classes

Members with mobility concerns are eligible to participate in the YMCA’s 'Tai Ji Quan™: Moving for Better Balance' classes, offered both virtually or in-person. You can join the 24-week program once per year at no-cost to you. To enroll, call Granite State YMCA at 603-232-8668 or send an email.

$10 reward for up to 10 prenatal visits*

Prenatal visits are an important part of staying healthy while you’re pregnant. Complete the form below and have your doctor sign it during each of your visits to receive $10 per visit (up to $100).

$40 reward for a portpartum visit*

Visit your doctor or midwife for a postpartum visit 1-12 weeks after giving birth. Must be a WellSense member at the time of the postpartum visit.

Sunny Start benefits

Sunny Start is a pregnancy program for expecting parents that offers additional support throughout the course of pregnancy and the postpartum period. Some of the benefits include childbirth class reimbursements, help getting breast pumps at no charge, and guidance on signing up for services like WIC.

**Please allow 10 to 12 weeks from the time you submit your request to receive your reimbursement. This form is to be used by WellSense members who were a member for at least 3 months in a calendar year prior to Sept. 1, 2026. You may use this form if you were a member during that time and are looking to request reimbursement for a fitness tracker, WeightWatchers® fees, or fitness club fees. You may be reimbursed for a fitness tracker –OR– a gym membership in the same calendar year, but not both. Reimbursement forms are due by March 31, 2027.
