A bridge to better days
At WellSense, we know that health is about more than medication or checkups. It’s also about stability, trust and being seen as a whole person. That’s what Miguel found when he met Olivia.
Miguel joined WellSense in early 2024 after several hospital stays for COVID-19 and a recurring kidney infection. He was also managing long-term health conditions including diabetes, high blood pressure and the effects of multiple strokes. On top of it all, Miguel did not speak English and felt overwhelmed and alone along his difficult health journey.
That changed when he connected with Olivia, a bilingual WellSense Care Manager. It took eight outreach calls before Miguel began to trust her, but Olivia didn’t give up.
She quickly discovered that with all of Miguel’s health and communication challenges, he was also struggling to manage his medications due to poor vision. This was affecting his ability to stay healthy and out of the hospital. Olivia provided education, encouragement and tools to help Miguel stay on track. As a result, he completed an important course of antibiotics—keeping him from any new admissions. He also learned how to manage his ongoing health conditions, and he now checks his blood sugar and blood pressure daily.
Transportation was another barrier to care for Miguel. Olivia helped him enroll in the WellSense transportation program, which allowed him to resume attending doctor’s appointments — an important step in staying on the right track. She also worked with his care team to request access to physical therapy and home health services for Miguel.
As Olivia got to know Miguel better, she learned that more than anything—more than medications, mobility or appointments—Miguel feared losing his work permit and immigration status, which had been impacted by his strokes. She worked with a WellSense Community Health Worker to connect Miguel with legal aid and a Spanish-speaking advocate, helping him take the first steps toward resolving his status and reapplying for insurance.
As a result of the connection Olivia has established with Miguel, he hasn’t been hospitalized in months, is staying on top of his health and is continuing his care with a WellSense Care Manager. He says Olivia gave him hope—and calls her “his angel.”
