Social Determinants of Health (SDoH) such as housing, food, income, education, and transportation have a profound impact on both behavioral and physical health. Up to 80% of health outcomes are shaped by these non-medical factors1.
Unmet social needs increase risk for mental health issues like depression and anxiety, and can worsen chronic conditions such as diabetes, heart disease, and asthma, particularly when behavioral health needs are also unaddressed2,3.
Integrated care that addresses SDoH alongside medical and behavioral health through screening tools, care coordination, and community partnerships improves outcomes and reduces preventable healthcare use. Recognizing SDoH is essential for delivering equitable, whole-person care.
