1. Lost your ID Card?

Log in to order a new ID card.   

2. Moved? Changed your phone number?  

Log in to update your account information.

3. Need to change your primary care provider?  

Once you choose a new primary care doctor, nurse practitioner, or physician assistant, let us know who your new primary care provider is.

4. Have special health needs?

Our Care Management program can make living with asthma, diabetes or other special health needs easier. Not sure if you have special health needs? Take the health risk assessment or call us, so we can help you get the right kind of care.

It’s important that we know who your primary care provider (PCP) is. If we don’t know who you’ve picked, we may assign you someone else. Call Well Sense Member Services at 877-957-1300 or update your information on the online portal.
