Who's the MVP of your healthcare team?

When it comes to your healthcare team, your PCP is your MVP. Your Primary Care Provider (PCP), also known as your personal doctor, is your healthcare quarterback. They call the plays by knowing your health history, recommending preventive care and coordinating care with specialists and other providers you might need.

If you already have a PCP, the beginning of the year is a great time to schedule your no-cost Medicare Annual Wellness Visit. You’ll review your overall health, medications and upcoming preventive care and screenings you should schedule.

You’re leaving the hospital. What’s next?

Four steps to prevent returning

There’s a lot to think about when being discharged from the hospital. Top of the list for most people is how to keep from coming back. Here are four things you can do to help make sure your hospital stay is one-and-done.

Get your instructions

Meet with a discharge planner before you leave the hospital. They will provide a set of specific instructions for you to follow.

Contact your doctor
Contact your personal doctor within 48 hours of being discharged from the hospital or emergency department to let them know about your hospital stay and how you are doing. If you and your provider have a patient portal, like My Chart, it’s easy to send your doctor a message through that platform.

Schedule an appointment
Schedule an appointment with your personal doctor within seven days of discharge from the hospital. You can always ask your doctor about doing a virtual visit from the comfort of your home.

Understand your medications
Make sure you understand what the medications you were prescribed are for and take them as instructed. Talk to your pharmacist or your doctor if you have any questions. Always take your prescriptions as instructed, even if you’re feeling better. Don’t stop taking any medications without first checking with your doctor.

  • Don’t know if you need to see a doctor, go to the emergency room or treat symptoms at home
  • Need help with taking your medications
  • Need nonemergency* care but your doctor can’t see you or it’s after hours
